In 2012, Patrick had a mental health breakdown and jumped off a cliff edge. He survived and was subsequently detained under the Mental Health Act and admitted into the care of Sussex Partnership NHS Foundation Trust. Patrick received insufficient and inadequate treatment and died by suicide at home less than three days after being prematurely discharged. This is an account of the events that led up to his death.
