Ovulation Induction gives an integrated view of basic ovarian physiology, pathophysiology, and the clinical management of ovulatory disorders. It provides core information upon which specialists can base clinical decisions. The most important feature is the inclusion of newer agents now available for ovulation induction. Newer clinical applications for older medications as well as various treatment combinations (including GnRH analogs) are thoroughly discussed. There is a frank discussion of risks, side effects, and the expense related to ovulation inducing agents. Many of the results in this book have never before been published in book form. Topics examined include the neuroendocrine regulation of the menstrual cycle, ovarian physiology and in vitro fertilization, the diagnosis of ovulation and the role of ultrasound in monitoring the follicular response, the use of clomiphene citrate for ovulation induction, and the role of bromocriptine and related compounds for ovulation induction in hyperprolactinemia.

In obstetrics and gynecology, as in other medical disciplines, great satisfaction comes from doing, but a greater satisfaction comes from knowing. The desire to know raises clinical practice to its highest level. This principle guided us in determining the objectives for this volume. Several standard textbooks of obstetrics and gynecology include information about infectious problems. Infectious Diseases in the Female Patient is unique in that it emphasizes primarily the basic science as pects of infections in obstetrics and gynecology. Although providing a practitioner's handbook was not our goal, the reader nevertheless will find discussions of the management of infections in the female pa tient. The continued growth of knowledge about infectious disease en countered in obstetrics and gynecology has come from many sources: bacteriology, virology, genetics, immunology, biochemistry, physiol ogy, and pharmacology. Insights from all these disciplines contribute to the conceptual framework of this volume. Many of the authors who contributed to this text are leaders in their respective areas of research and bring to the book a diversity of ex pertise and experience. Controversial issues are developed in a bal anced fashion, and, in most cases, opposing views are discussed. Since medical texts usually are read piece-meal rather than cover to cover, we have allowed some overlap between chapters. The slight degree of redundancy allows each chapter to stand on its own, so that the reader may obtain the necessary information from a single chapter.

On the other hand, there are fears and misconceptions. I have heard many doctors say "I have never been sued for a section I did, but I have been sued for the section I did not do. " The fear of not having performed a section in my opinion is real, although difficult to prove, and until the public can be educated that cesarean section delivery cannot eradicate fetal death and damage, this fear will remain and will be responsible for some unnecessary cesarean sections. Bruce Flamm and I hope this book will correct misconceptions that have been responsible for many unnecessary cesarean sections. I am still frequently asked the same old question: What is an ideal cesarean section rate? I still give an answer similar to the 1983 answer, perhaps somewhat modified.
