Chest sonography is an established procedure in the stepwise imaging diagnosis of pulmonary and pleural disease. It is the method of choice to distinguish between solid and liquid lesions and allows the investigator to make an unequivocal diagnosis without exposing the patient to costly and stressful procedures. This book presents the state of the art in chest investigation by means of ultrasonography. A number of excellent illustrations and the compact text provide concise and easy-to-assimilate information about the diagnostic procedure. Basic elements such as indications, investigation techniques and image artifacts are detailed in separate chapters.

Biopsy Pathology of the Bronchi is intended to provide the practicing pathologist with a convenient source of diagnostic information, combined with pertinent clinical patterns and a background of histogenesis. It is hoped that the book will facilitate the accuracy of diagnosis, minimize pitfalls of interpretation and assist in the rapid evaluation of acute and chronic bronchial lesions, including neoplasms. Biopsy is taken in the widest sense and the book includes discussions on cytology, microbiopsies (fine needle aspiration biopsies, curette samples, and biopsies obtained by microforceps) and macrobiopsies, obtained at the time of surgery. Part I of the book deals with the procurement and preparation of bronchial specimens and includes a chapter on fetal development and structure of the normal adult epithelium. In Part II, the histopathology of numerous bronchial diseases are described in detail. Emphasis is placed on the diagnostic features of human lesions' which are illustrated by numerous high quality light and electron micrographs. Understanding normal bron chial structure, cell kinetics, and modulations of phenotypic expression provide insight into many pathological processes. Therefore, although emphasis is placed on the diagnostic features of human disease, the histopathology of corresponding lesions in experimental animals is dis cussed where this provides information helpful in the interpretation of the human lesions. This book presents state-of-the-art pre-clinical models and clinical applications of stem-cell-based therapies applied to different lung diseases, with a special focus on the translation of bench data into clinical studies. Starting with the assumption that abnormal lung tissue repair and regeneration has emerged as the driving force underlying pathogenesis and progression in many lung diseases, it sheds new light on the potential of stem/stromal cells as drivers of repair and sources of reparative factors in the lung. The first part of the book offers an overview of stem cell types and mechanisms involved in lung development, homeostasis, repair and regeneration, and reveals the crucial role of the extracellular matrix within the lung microenvironment. In the second part, leading experts present the latest pre-clinical evidence and clinical applications of stem-cell-based therapies in a wide variety of lung diseases, ranging from COPD and lung fibrosis to other rare lung diseases. The last section discusses stem cell delivery systems and devices, such as aerosolised spray application. This book appeals to pneumologists, stem cell and matrix biologists, as well as bioengeneers with a special interest in regenerative medicine applied to pulmonary diseases.

This book provides an overview of key issues with regards to implementing telemedicine services as well as an in depth overview of telemedicine in pulmonary, critical care, and sleep medicine. Topics range from specific practices to program development. Telemedicine has experienced explosive growth in recent years and yet, implementing telemedicine solutions is complex with substantial regulatory, legal, financial, logistical, and intra-organization/intra-personal barriers that must be overcome. This book provides the necessary information and guidance to address those complex issues. This book is broadly divided into two parts 1) a primer on requisite steps before embarking on telemedicine service development and 2) specific applications and examples where telemedicine is successfully utilized to improve quality of care in pulmonary, critical care, and sleep medicine. The first part includes coverage of telemedicine and finance, regulatory and legal issues, and program development. The second part delves into specifics with information on ambulatory telemedicine programs, inpatient consultations, and tele-ICU programs. All chapters are written by interprofessional authors that are leaders in the field of telemedicine with extensive knowledge of diverse telemedicine programs and robust real-world experience on the topic. This is an ideal guide for telehealth program managers, and pulmonary, critical care, and sleep medicine professionals interested in improving their telehealth practice.

Pulmonary embolism (PE) has experienced a rapid expansion in available treatments, from hyperacute emergency care to the detailed investigation of persistent breathlessness despite anticoagulation during follow up. Whilst recent clinical practice guidelines provide a robust evidence base for more routine aspects of pulmonary embolism management, clinicians frequently face patient-specific challenges where the evidence for how to proceed with patient management may be less secure.Derived from the personal experience of expert clinicians who are engaged in all aspects of PE care, this book provides a practical update on contemporary management aspects from life-threatening presentation of pulmonary embolism with failed thrombolysis to the patient presenting with complex comorbidity or during pregnancy. General physicians and clinical specialists interested in contemporary diagnosis and management of acute pulmonary embolism will benefit from this book.

Much research over the past 30 to 40 years has shown that the inflammatory response, while critical for host defense during microbial infection, may itself play a central role in the pathogenesis of sepsis. Although key mediators responsible for this injury have been identified, efforts clinically to augment our conventional antimicrobial and supportive therapies during sepsis with agents modulating the inflammatory response have been unsuccessful. As a result, the mortality associated with this lethal syndrome, especially when complicated by shock, has remained persistently high. Unfortunately, during this same period of time, the incidence of sepsis has accelerated as other fields of medicine have relied increasingly on therapies that predispose to infection. While frustrating, overall this experience in the field of sepsis has not been without value. Most importantly, it has helped define on several different levels the complexity of the septic patient. Recognizing and addressing this complexity as discussed by each of the contributors to Evolving Concepts in Sepsis and Septic Shock may now provide new inroads into the treatment of sepsis.

This book is a guide to the research, findings, and discussions of US and international experts on climate change and respiratory health. Since the publication of the first edition, climate change has been increasingly acknowledged as being directly related to the prevalence and incidence of respiratory morbidity. Evidence is increasing that climate change does drive respiratory disease onset and exacerbation as a result of increased ambient and indoor air pollution, desertification, heat stress, wildfires, and the geographic and temporal spread of pollens, molds and infectious agents. This second edition is fully updated to include the latest research by international experts on topics such as heat waves causing critical care-related diseases, climate-driven air pollution increases, and high-level ozone and ozone exposure linked to idiopathic pulmonary fibrosis, lung cancer, and acute lower respiratory infection. Seven new chapters have also been added on extreme weather and agricultural safety in California; desert dust effects on lung health; climate policy and the EPA; California's integrated approach to air quality and climate change; integrating climate change, the environment, and sustainability themes into professional health science courses; and the role of the physician as climate advocate. This is an ideal guide for all pulmonologists and health professionals treating patients with pulmonary disease.
