Dr. Heinonen reviews and critically evaluates the scientific literature on the biological role of inorganic pyrophosphate (PPi ) published from 1940 to the end of 1999. He describes and classifies all known biochemical reactions that produce Ppi; describes and evaluates all published methods used in biological Ppi; and compiles and critically evaluates information on the concentration of PPi (with the conclusion that, contrary to common belief, PPi exists throughout the living world in rather high concentrations). Many reactions in which PPi is used as a biochemical energy source instead of ATP have been described in recent decades, especially in bacteria, protists, and plants. These reactions are evaluated from the bioenergetic and regulatory points of view. Also considered is the possible role of PPi as a source of biochemical energy in the primitive phases of life, before ATP. Data is presented on the regulatory role of PPi in living systems, such as activities of enzymes, fidelity of syntheses of macromolecules, and proliferation of cells. PPi may also regulate the formation and dissolution of bone as well as pathologic calcification of soft tissues and the formation of urinary stones. The formation of calcium pyrophosphate dihydrate crystals in the extracellular fluids of joints cause the disease called pseudogout. Biological Role of Inorganic Pyrophosphate book is a unique and invaluable source of references (about 1120) and summarized data for professionals who study or plan to study the role of PPi in living systems. Many different branches of science (biochemistry, microbiology, bioenergetics, plant physiology, parasitology, evolution, orthopedics, rheumatology) have involvement with PPi. This book sums up available knowledge in one place and will help scientists cross disciplinary boundaries.
An in-depth analysis of hand surgery principles from internationally renowned hand surgeons Hand surgery is a multidisciplinary pursuit that requires exceptional artistry, superb skills, and harmonious teamwork developed over decades of practice. Reconstruction of the Thumb and Finger is a unique resource that follows a Socratic approach to teaching in a world consumed by computers. It reflects 225 years of collective surgical effort by five prominent hand surgeons, who have practiced from the embryonic stages of pedicle flaps and digital transposition to revascularization and free tissue transfers. The differing goals, techniques, expectations, and collaborative approaches advocated by these masters are articulated throughout this exceptional book. The first chapter provides a comprehensive overview of anatomical and functional eloquence of the hand, including thorough descriptions of the musculoskeletal system, principles of thumb and digit reconstruction, and a detailed classification of defects. Traumatic loss and congenital differences of the thumb and finger are systematically divided into specific levels in 20 chapters. At each interval, various methods of reconstruction are presented with an emphasis on both new and older techniques. Chapter 20 covers elective incisions of the finger, hand, wrist, and forearm. Key Highlights A comparative digital defect indexing system (DDI) based on the surface involved, the critical zone of injury or loss, and a sum of tissue loss From simple to complex, each case includes the treatment rationale, functional/anatomical loss, reconstruction objectives, multiple treatment options, technical provisos, long-term outcomes, and clinical pearls Older clinical cases with long-term data bridge the gap from older to current microvascular techniques Pertinent anatomy is artfully illustrated by one of the recognized founding fathers of hand surgery, the late J. William Littler, including unpublished drawings and classic illustrations Reflecting many decades of research and collaboration, this authoritative medical reference is indispensable reading for novice to seasoned hand surgeons and microsurgeons. It also provides a robust teaching tool for medical students and allied health personnel.
This book provides detailed information on functional anatomy, physical examination, and clinical radiology of the knee with a view to enabling the clinician to identify the most suitable treatment approach to different knee joint pathologies. In addition, the arthroscopic treatment techniques most frequently employed in patients with these conditions are described, with presentation of numerous arthroscopic images detailing characteristic findings. Knee joint pathologies today represent a significant challenge owing to the complexity of the injuries suffered, rising activity levels, and high patient expectations. A proper physical examination plays an important role in diagnosis. The surgeon who has the opportunity to conduct a clinical evaluation must fully understand the role of radiological evaluations, and assessment by a radiology expert is also necessary. In all cases, knowledge of the normal anatomy and its correlation with clinical and radiological findings is fundamental to correct diagnosis and treatment selection. Surgeons and trainees with an interest in knee joint pathologies will find this book to be an excellent, richly illustrated educational guide to the subject.
Now in a fully revised and expanded second edition, this practical text presents the current state of the art and latest advancements in the biomechanics, assessment, diagnosis and management of UCL injury in the elbow. In the years since this book¿s initial publication, significant developments have occurred on multiple fronts relating to elbow UCL injury, including injury prevention, less invasive repair techniques, more anatomical surgical reconstructions, and improved post-injury rehabilitation protocols. Chapters are once again arranged thematically, beginning with discussion of the relevant anatomy and surgical approaches, throwing biomechanics and overload mechanisms, epidemiology, history and physical exam. After a description of the radiological approaches to assessment, both conservative and surgical strategies are outlined and discussed in detail, from repair both with and without augmentation to reconstruction both arthroscopically and with newer minimally invasive techniques. Considerations for UCL injury in special populations ¿ the young athlete and the female athlete ¿ and sports-specific rehabilitation, return-to-play and prevention via wearable technology round out this thorough presentation. Enhanced with select video clips illustrating surgical techniques, Elbow Ulnar Collateral Ligament Injury, Second Edition remains a go-to resource for orthopedic surgeons, sports medicine specialists, therapists and trainers who work with athletes that suffer from these conditions.
This book offers a comprehensive and detailed overview of specific sports-related injuries and a valuable guide for decision-making to establish the best strategies to prevent and manage such injuries. As a thorough understanding of each sports modality plays a key role, both in injury prevention and management, a dedicated chapter is devoted to each sports discipline. An international panel of authors examines all most popular individual and team sports ¿ including athletics, swimming, combat sports, cycling, tennis, American football, baseball, basketball, soccer and volleyball, just to mention a few. Three additional chapters present special aspects related to sports injuries: mental health concerns in athletes, radiological assessment and patient reported-outcomes tailored to sports medicine. All chapters share a consistent format, starting with a brief presentation of the sport and its history, and then discussing its dynamics, physical demands on the athlete, common sports-related injuries, biomechanics of injuries, first aid on the field, and injury prevention. This book describes and discusses the available joint preservation techniques for maintaining the stability homeostasis of the lower extremity joints ¿ specifically the hip, knee, and ankle ¿ following injury. Readers will find detailed coverage of anatomy, pathology, techniques for repair, restoration, and regeneration, and rehabilitation strategies. Joint preservation is an emerging field in Orthopaedics that represents a response to the limitations of joint replacement technology. Using the techniques now available, surgeons can try to prevent or delay the onset of osteoarthritis or other degenerative conditions affecting the joints, particularly in young patients. Furthermore, modern tissue engineering offers the potential for whole-joint resurfacing, thereby achieving complete restoration. Optimal implementation of these techniques depends upon further refinement of methods and continuing improvements in knowledge of biomechanics, biology, and anatomy. Against this background, the present book is an ideal guide to the latest treatment modalities that will appeal to all who wish to learn more about indications, goals, procedures, and expected outcomes.
