The role of the gastrointestinal (GI) tract in the maintenance of health and prevention of disease has been a concept proposed for thousands of years. However, until recently our understanding of the role of the GI tract was predominantly limited to digestion and absorption. The importance of the intestinal barrier function of the GI tract was not well understood due to limitations in medical technology, resulting in less research being conducted in this area. Increased intestinal permeability (IP), referred to as "leaky gut" in lay terms, was considered a pseudoscience. Until recently, clinicians referring to increased IP as a real occurrence would have had their credibility questioned [1, 2]. Today, the importance of the GI tract in the development and management of intestinal and extra-intestinal conditions is receiving significant interest from researchers, clinicians and the general population. Increased IP is now recognised as a real phenomenon that contributes to the development of conditions ranging from inflammatory bowel disease to non-alcoholic fatty liver disease, with research in the area rapidly growing and evolving [2, 3].

Cells may die by a number of mechanisms, whether it be to the detriment or benefit of the organism. This can be external to the control of the cell, or initiated and executed by the cell itself, in response to various stimuli. Programmed cell death, when the cell regulates its own demise in response to specific stimuli, may occur through processes such as apoptosis, necroptosis and pyroptosis (D'Arcy, 2019). The most commonly investigated form of programmed cell death to date is apoptosis, which was first described in 1972 (Kerr et al., 1972). Further advances have been made in the understanding of apoptosis since then, stemming from investigation of programmed cell death in C. elegans (Horvitz, 1999). Its evolutionary conservation as a mechanism of cell death across diverse species, ranging from the aforementioned nematodes to mammals, indicates the ironic importance of this process in the maintenance of life (Severin et al., 2008). Apoptosis is important in both health and disease, regulating embryonic development and displaying polarising roles in pathologies such as cancer and sepsis (Glab et al., 2017b). As such, apoptosis can be both harmful and beneficial to an organism. Additionally, unlike sporadic cell death mechanisms like necrosis, the orderly manner of apoptotic cell disassembly enables the process to go largely undetected by the immune system and avoid initiating inflammatory responses that may otherwise damage the surrounding tissue (Rock and Kono, 2008). Apoptosis can be initiated in response to a variety of stimuli, both intra- and extracellular in origin (Danial and Korsmeyer, 2004). These then activate one of two distinct pathways, ultimately converging on the death of the cell

"Before the discovery of insulin, diabetes was treated almost exclusively through diet, from subsistence on meat, to reliance on fats, to repeated fasting and near-starvation regimens. After two centuries of conflicting medical advice, most authorities today believe that those with diabetes can have the same dietary freedom enjoyed by the rest of us, leaving the job of controlling their disease to insulin therapy and other blood-sugar-lowering medications. Rather than embark on "futile" efforts to restrict sugar or carbohydrate intake, people with diabetes can lead a normal life, complete with the occasional ice-cream cake, side of fries, or soda. These guiding principles, however, have been accompanied by an explosive rise in diabetes over the last fifty years, particularly among underserved populations. And the health of those with diabetes is expected to continue to deteriorate inexorably over time, with ever-increasing financial, physical, and psychological burdens. In Rethinking Diabetes, Gary Taubes explores the history underpinning the treatment of diabetes, types 1 and 2, elucidating how decades-old research that is rife with misconceptions has continued to influence the guidance physicians offer--at the expense of their patients' long-term well-being. The result of Taubes's work is a reimagining of diabetes care that argues for a recentering of diet--particularly, fewer carbohydrates and more fat--over a reliance on insulin. Taubes argues critically and passionately that doctors and medical researchers should question the established wisdom that may have enabled the current epidemic of diabetes and obesity, and renew their focus on clinical trials to resolve controversies that are now a century in the making."--Publisher marketing.

In 2018 I was diagnosed a type-2 Diabetic. I got out a bound notebook and began documenting my situation. My past glucose was above the glucose normal range of 74-100 mg/dL, my HbA1C 11.5%, my weight was 185 lbs. and my height 5 feet 11.5 ins. I was already using Dr Amy LEE's, probiotic BIO-X4. Dr, Lee is a Nutritionist and Weight Management Specialist. A side benefit of her program was my HbA1C dropped to as low as 3.5%. I was monitoring my finger-stick blood glucose levels preBREAKFAST, preLUNCH and preDINNER. On February 15th I started with Dr Marlene Merritt's Blood Glucose Formula (BGF) to see if this probiotic would indeed lower my circulating blood glucose. The preLUNCH and preDINNER began to separate from the preBREAKFAST (nocturnal) glucose values. This glucose change was statistically significant and the preDINNER glucose levels separated statistically faster led me to identify a nocturnal phase and a diurnal phase of Diabetes. I was able to verify and extend the finger-stick data with the LibreLink technology. Now this single Type-2 Case Study needs to be verified with ten (5 female/5 male) patients and extended to nondiabetics and severe diabetic groups for comparison, and a better understanding of Diabetes. Glucose is a universal energy molecule that can be found in every cell in the body, and maintaining its normal levels is critical to good health.
