Thanks to the breast cancer research of recent decades, effective methods and strategies have been established, allowing the mortality rates of breast cancer patients to decrease more and more. However, we still face many challenges. The role of certain biomarkers or mutations is still not clear for treatment and surveillance. Novel drugs are often associated with a different spectrum of adverse events than that seen for conventional therapies, accordingly having an impact on patients' compliance behaviors. In breast cancer surgery, in some cases, the question remains whether to escalate or deescalate. Additionally, with regard to diagnostics, in a digitalized world, home-based tools and therapy monitoring options are of high importance, especially under the tough conditions and supply problems seen during the COVID-19 pandemic.This Special Issue gathers original research articles and reviews demonstrating therapeutic challenges, current research strategies, and novel diagnostics in breast cancer.
Glaucoma is a major cause of blindness that can be prevented if detected and treated in the early stages of the disease. Viruses in the Parvoviridae family constitutes a most diverse and intriguing field of research. Parvoviruses have a long evolutionary history and, beneath an apparent simplicity and uniformity, can differ widely in structure, genome organization and expression, virus-cell interaction, and impact on the host. The advent of optical coherence tomography (OCT) has arrived in an era in which the in vivo, cross-sectional imaging of both the retina and choroid is possible. Infra-red rays of light are used to image the chorioretinal layers with an image resolution reaching 10-15 microns. During the last three decades, OCT technology has undergone several improvements, such as spectral domain OCT (SD-OCT), swept source OCT (SS-OCT), enhanced depth imaging (EDI-OCT), wide field OCT, intraoperative OCT, OCT angiography (OCTA), and adaptive optics OCT. These enhancements help in both the qualitative and quantitative analysis of chorioretinal parameters in pathologies such as macular edema, neurosensory detachment, retinoschisis, macular holes, epiretinal membranes, and choroidal tumors. An added advantage is the role of OCT in the evaluation of optic disc disorders, including optic neuritis and glaucoma, via the quantitative analysis of the retinal nerve fiber layer. Moreover, OCT being non-invasive provides an opportunity for serial imaging during follow-up visits. Overall, OCT imaging provides tissue details, closely mimicking the histopathology, and is an invaluable asset in the armamentarium of retina physicians. This special issue includes applications of OCT in various chorioretinal disorders.

Chronic obstructive pulmonary disease (COPD) is a disorder characterized by airflow limitation and is one of the major causes of mortality and morbidity across the globe. Although several guidelines have been published over the past three decades, they were generated in response to the fact that COPD is a major cause of mortality and morbidity and remains an important social problem. In the 20th century, forced expiratory volume in one second (FEV1) was the single most important measure in patients with COPD from discriminative, evaluative, and predictive standpoints. We found that dyspnea is a better mortality predictor than FEV1 in 2002 and exercise capacity in 2003. Subsequently, it has become apparent that physical activity is more predictive of mortality. Furthermore, we have recently developed the tendency to use the reduction of the future risk of exacerbation as the endpoint of relatively large-scale clinical trials rather than the improvement of FEV1. Many researchers have continued to seek better outcome markers and have also discussed what should be used as primary or secondary endpoints in clinical trials in subjects with COPD. We have undoubtedly made great progress but still have a lot of work to do for the best interests of patients.

This book covers the tremendous progress in the current understanding of the molecular physiology of voltage-gated calcium channels. This book includes unparalleled insights into structural features of calcium channels due to X-ray crystallography and cryo-EM, which in turn yielded critical information into how these channels function under normal and pathophysiological conditions, and how they interact with calcium channel therapeutics. The chapters investigate how, with the advent of high throughput genome sequencing, numerous mutations in various calcium channel genes have been identified in patients with neurological, cardiovascular, neuropsychiatric and other disorders. This is further complemented through a much larger in vivo toolkit such as knock-out and knock-in mice. The chapters further discuss the increased complexity of calcium channel physiology that arises from mRNA editing and splicing. Finally, the book also provides an overview of the updated research on calcium channel inhibitors that can be used both in vivo and in vitro, and which may serve as a spring board for new calcium channel therapeutics for human disease.
