Yet, the metrics necessary to evaluate the quality and assurance of the healthcare is a matter subject to continuous debate. In the recent past, the concerns associated with the quality of care has attracted wide-ranging interests, particularly by the Mid Staffordshire Hospital scandal. An inquiry report into the Mid Staffordshire NHS Foundation Trust, the Francis Report, released on February 2013 paints a grim picture of the events that transpired during fermentation of the scandal. Unfortunately, these events led to the loss of lives, due to issues which could have been managed or addressed better. Briefly, the Stafford and Cannock Chase hospitals decided to pursue the Foundation Trust in 2005, which was subsequently granted in 2008. However, abnormally high death rates raised eyebrows from various quarters, triggering the formulation of the taskforce to look into the matter. It is largely believed that the scandal was triggered by the quest of the managers to cut down the operational costs and accomplish the minimum labour requirements in attempts to achieve the foundation status and qualify for the NHS trust funds. This indicates that the scandal was as a result of catastrophic organisational challenges that were triggered by poor change management.

Dem ungeachtet, sind sie auch einer wirtschaftlichen Arbeitsweise verpflichtet. Bei immer knapper werdenden finanziellen und personellen Ressourcen bzw. der zukünftigen demographischen Entwicklung und um die Qualität in der Pflege zu halten oder diese zu verbessern, ist es notwendig, die bestehenden Systeme weiterzuentwickeln und den modernen Erfordernissen anzupassen.Diese Facharbeit beschäftigt sich mit dem Thema der ¿Implementierung der Strukturierten Informationssammlung im ambulanten Pflegedienst¿, im folgenden SIS abgekürzt. Da wir in unserem Unternehmen, einem ambulanten Pflegedienst mit ca. 100 Patienten, die SIS in der zweiten Hälfte dieses Jahres einführen möchten und die Implementierung des neuen Systems in meinen zukünftigen Zuständigkeitsbereich als Pflegedienstleitung fallen wird, habe ich mich für dieses Thema entschieden. Ein weiterer persönlicher Motivator zu diesem Thema ist, dass ich diesen Strukturwechsel im Kontext der Anforderungen an die Einrichtungen, als Antwort auf die demographischen Entwicklungen im Bezug zu den finanziellen und pflegerischen Ressourcen, als systemimmanent sehe. Die bürokratischen Anforderungen an einen ambulanten Pflegedienst setzen einen engen Rahmen und binden wichtige Ressourcen, welche durch die Entbürokratisierung in der Pflege, nicht einfach nur entlastet werden sollen, nein sie sollen zielgerichteter den tatsächlichen Bedürfnissen und Anforderungen der Patienten Rechnung tragen und dabei ein elementarer Bestandteil der Qualitätssicherung in der Pflege bleiben.Im ersten Teil meiner Facharbeit beschäftige ich mich mit dem neuen ¿entbürokratisierten¿ System. Im weiteren Verlauf meiner Facharbeit gehe ich auf die Frage ein, welche Auswirkungen die Implementierung der SIS für die Mitarbeiter hat. Hier setze ich mich mit einer Theorie zu Veränderungsprozessen auseinander. Im darauffolgenden Teil entwickle ich ein System, mit dem die SIS in unserem Pflegedienst eingeführt wird und wie die daraus resultierenden Veränderungsprozesse bei allen beteiligen Mitarbeitern zielführend begleitet werden können.

Der Fokus dabei liegt auf der ambulanten und der stationären Versorgung von Patienten und es wird der Frage nachgegangen, ob es Unterschiede in der Aufgabe des CM gibt. Case Management im stationären Bereich hat sich mittlerweile etabliert, während im ambulanten Bereich der Versorgung sich das Case Management auf Grund verschiedener Faktoren nur langsam entwickelt. In der spezialisierten ambulanten palliativen Versorgung (SAPV) wird der Case Manager meist nicht als solcher benannt, sondern trägt den Namen Koordinator. Aber was genau sind die Aufgaben eines ambulanten Case Managers im Bereich der Palliativversorgung und wo gibt es Unterschiede zu dem Arbeitsbereich des stationären Case Managers? Ist der Koordinator weniger als ein Case Manager und übernimmt tatsächlich nur die Aufgabe der Koordination, oder verbirgt sich hinter dem Begriff der Koordination doch mehr an Aufgaben. Macht es dann ggf. Sinn diesen Begriff in Zukunft auszuweiten, umzubenennen etc. Im Rahmen dieser Arbeit soll der oben genannten Frage nachgegangen werden, indem zunächst das Fachgebiet der Palliative Care und dessen Aufgaben, Herausforderungen und Ziele beschrieben werden. Im Verlauf wird der Fokus dann auf den Bereich der ambulanten Versorgung und der daraus resultierenden Unterschiede im Vergleich zum stationären Case Management im Bereich der Palliativversorgung und auch den besonderen Herausforderungen im Vergleich zu anderen Fachgebieten gelegt. Hierfür werden die Rollen und Aufgaben des Case Managements näher beleuchtet.Am Schluss soll dann die Frage beantwortet werden, welche Aufgaben den Koordinator genau von anderen Case Managern unterscheidet und ob es wirklich Unterschiede innerhalb des Fachgebietes gibt oder ob es lediglich an den allgemeinen Strukturen der stationären oder ambulanten Versorgung hängt.

Leadership seems to have become a powerful tool in organizational management. This is attributable to the notion that leaders, rather than managers have significant influences in legitimate organizations. According to Bertocci (2009), one of the key reasons why leadership has become an integral element in recent years is because of the evolving nature of the business world which has become more competitive, more global and more volatile. It is also apparent that organizations are increasingly becoming technologically complex; thus, demanding a high level of expert power. Given the evolving nature of today¿s workplace environment and dynamics in the business world, it is explicit that leadership enhances organizational effectiveness through improving production, flexibility, competitiveness, quality, efficiency, satisfaction, and development.This is consistent with the opinion held by social scientists and organizational management experts who believe that effective leadership results into a level of order and consistency to the profitability, as well as quality of products and services. On the other hand, Lunenburg (2012) observes that a true leader promotes organizational effectiveness through influencing others and modifying their behavior. In retrospect, leadership has not been a complex task, but the evolving workplace environment due to changes in organizational structure and design are increasingly driving evolution in leadership. In order to cope with the current organizational change, it is believed that effective leadership is essential for organizational survival.

Diploma Thesis from the year 2017 in the subject Health - Nursing Science - Nursing Management, grade: B, , course: DIP NURSING, language: English, abstract: The main purpose of this study was to find out the factors that influence the use of the nursing process in patient care by nurses at Nyamira level five hospital. Both the factors that promote the nursing proves and also the factors that hinder the using of nursing hugely affect the outcome of nursing care that the patient receives either positively or negatively. The literature review provides the background information about the topic under investigation. A descriptive survey method was used for the study. Data was collected using a questionnaire. Stratified random sampling was used to select a sample size of 108 respondents from the target population. Analysis of data rely heavily on statistical tools which enable one to come up with better presentation of the findings using pie charts and tables.
