Case Management ist im Krankenhaus ein wichtiges Instrument, um die weitere Versorgung von Patienten (Nutzer) im häuslichen Bereich oder Seniorenheim zu gewährleisten. Ein nahtloser Übergang für den Patienten ist äußerst wichtig.Case Manager/in sind Fachausgebildet nach der DGCC (Deutsche Gesellschaft für Care- und Case Management) und viele Absolventen haben im Vorfeld in der Pflege gearbeitet. Diese Voraussetzung ist sehr wichtig. Als ausgebildete Pflegefachkraft hat man das medizinische und das Pflegerische Fachwissen. Das heißt, als Case Managerin hat man einen ganz anderen Blickwinkel, was den Versorgungsbedarf betrifft.Im Case Management geht es um den Patienten, der sich zurzeit in einem Ausnahmezustand befindet, aufgrund einer akuten Erkrankung oder durch eine wesentlichen Verschlechterung der chronischen Erkrankung. Das bedeutet für den Patienten gravierende Einschnitte der gewohnten Lebenssituation und Einschränkung in der Lebensqualität durch Selbstpflegedefizite. Sie haben Ängste, sind angewiesen auf Angehörige, Pflegedienst und Home Care, sowie Antragstellung bei der Krankenkasse, Pflegekasse, Gespräche mit dem Hausarzt und Sanitätshaus und all das in verschiedenen Settings. Und genau da setzt auch das Case Management an. Der Patient (Nutzer)/ Bezugspersonen müssen über die rechtlichen Aspekte informiert werden. Was sind die Leistungen der Krankenkasse und welche der Pflegekasse? Was steht dem Nutzer zu? Auch die Pflegeperson kann Leistungen einfordern. Es gibt die Möglichkeit der Familialen Pflege. Bezugspersonen im häuslichen Bereich zu schulen. Die Pflegekasse stellt ebenfalls Schulungsprogramme vor Ort für Angehörige zu Verfügung, aber das muss die Pflegeperson erstmal wissen.Das Case Management optimiert und koordiniert einen auf den Patienten (Nutzer) abgestimmten Versorgungsprozess und vernetzt alle Akteure, die in der Behandlung des Patienten (Nutzer) involviert sind. Durch die intraprofessionelle Zusammenarbeit wird ein gezielte poststationäre Versorgung für den Patienten (Nutzer) und Angehörigen sowie für die stationäre Einrichtung (Seniorenheim / Kurzzeitpflege) oder den ambulanten Pflegedienst gewährleistet. Case Management bedeutet Fallmanagement, Netzwerkerstellung und Überwindung der starren Sektoren. Case Management ist auf den Einzelfall ausgerichtet. Das Ziel ist eine Verbesserung der Versorgungssituation durch die individuelle Fallbegleitung. Es bedeutet somit Schnittstellenmanagement und eine sektorenübergreifende Kommunikation und Vernetzung zwischen den Professionen. Ein auf den Patienten abgestimmtes Netzwerk muss gemeinsam mit dem Patienten (Nutzer) oder Bezugsperson geplant werden, um Versorgungsbrüche im häuslichen Bereich zu vermeiden.
