Was sind die Leistungen der Krankenkasse und welche der Pflegekasse? Was steht dem Nutzer zu? Auch die Pflegeperson kann Leistungen einfordern. Es gibt die Möglichkeit der Familialen Pflege. Bezugspersonen im häuslichen Bereich zu schulen. Die Pflegekasse stellt ebenfalls Schulungsprogramme vor Ort für Angehörige zu Verfügung, aber das muss die Pflegeperson erstmal wissen.Das Case Management optimiert und koordiniert einen auf den Patienten (Nutzer) abgestimmten Versorgungsprozess und vernetzt alle Akteure, die in der Behandlung des Patienten (Nutzer) involviert sind. Durch die intraprofessionelle Zusammenarbeit wird ein gezielte poststationäre Versorgung für den Patienten (Nutzer) und Angehörigen sowie für die stationäre Einrichtung (Seniorenheim / Kurzzeitpflege) oder den ambulanten Pflegedienst gewährleistet. Case Management bedeutet Fallmanagement, Netzwerkerstellung und Überwindung der starren Sektoren. Case Management ist auf den Einzelfall ausgerichtet. Christian and other nurses in the hostile modern and increasingly secular age may feel helpless in an environment that created the Liverpool Care Pathway (LCP) and continues to foster end of life 'care' through sedation and dehydration.
